Soniya Gharti Magar (GM)
Frail 16-year-old woman from Raspurkot, Pyuthan district with rheumatic heart disease (RHD). She was identified by cardiologists as an RHD patient in the screening camp. She had never known about the disease before and was shocked when she found out. She did not have a family history of RHD. She has 10 family members and is the first child in the family. Her family’s financial condition is below average. She was admitted to Shahid Gangalal National Heart Center on 20 June, 2022. On admission, she had severe PS (peak difference 179 mmHg), with mild TRK with moderate PAH (estimated PASP. calculated: 47+10 =57 mmHg) and is being admitted for PV balloons and half-closed ASD II. Her transesophageal Echo performed at the hospital showed an intact IAS. Her operation for Balloon PV was carried out on June 21, 2022 and was successful. Her intervention was uneventful. She was placed on a medication Clavam 625 mg PO tds for 9 more days and tablet Pantocid 40 mg PO OD. She was asked to follow-up on Medical OPD in SOS.
She and her family were very much thankful for the support provided by NHF and was very much satisfied with the service. She stated that after her recovery, in the future, she would like to be a nurse and help the people in need.
Dil Kumar Bishwokarma (BK)
is a 15 year old young man from Dhanusha. He is a student identified with ASD during screening event in Dhanusha district on 11 July, 2022. He is studying in 6 grade at Shree Sankat Mochan Middle School Ramrati Dev Sharan and is good with his studies. He had some difficulties with his activities but was unaware of heart disease. His parents don’t know either. They had never been to the cardiology department for a check-up. During the screening event at his school, it was discovered that he had ASD. His parents and teachers were informed at school about his illness and asked them to visit to Kathmandu for surgery. His family’s financial situation was not very good so they were reluctant to visit to Kathmandu. The cardiologist advised them thoroughly about the disease and assured them that there would not have any financial burden for the treatment, as all medical expenses will be covered by the NHF. They will only have to cover the cost of transportation. Finally they were convienced to visit Kathmandu for surgical interveniton.
The patient was admitted to the Shahid Gangalal National Heart Center on 4 August, 2022. On 5 August 2022, investigations were carried out and found to be: large secundum ASD 1.5 cm, left to right shunt, Mild TR with normal PA pressure, dilated RA, RV and PA. AO:2.7, LA: 2.8, LVEF: 65%, LVID D/S: 3.9/2.7 cm.
ASD device closure was done on August 5, 2022. Procedure was uneventful. The patient stayed in hospital for four days. Echo was done on the date of discharge with the findings s S/P Asd Device closure (ASO 16 mm on 5 August, 2022), ASD device in situ, no obstruction to pulmonary veins, SVC, IVC, Tricuspid valve and mitral valve, no MR, Mild TR with Normal PAP. The patient was discharged with IVF, tablet aspirin, INJ. Cefazolin, tablet Amilax, tablet Pantop, tablet Clavam, tablet Flexon. He was asked for follow up after 1 month or SOS. His family were very pleased with the results of this surgery. They expressed their gratitude for this humanitarian work. They even said that the NHF saved their son’s life. If he had not identified by the NHF in time, they would have difficult time to treat his disease.
